is characterised by very rapid, chaotic and ineffective atrial beats when
atrial contractions are virtually absent. In this situation the ventricles are
driven at irregular intervals giving the irregular
pulse (very important diagnostic sign). The ECG shows no change in the QRS
complexes and an absence of P waves;
sometimes the baseline shows irregular fibrillation waves. Common causes of
atrial fibrillation include coronary artery disease (especially after acute
myocardial infarction), valvular heart disease (especially mitral valve
disease) and cardiomyopathy (weakness of the heart resulting from a primary
abnormality in the myocardium,
usually of unknown cause). Atrial
fibrillation may be even asymptomatic, and patients with paroxysmal atrial
fibrillation are usually more aware of the arrhythmia than those in whom it is
established. Atrial fibrillation frequently precipitates or aggravates cardiac
failure in those with an abnormal heart, particularly those with mitral
stenosis or poor left ventricular function since atrial contribution to
ventricular filling is missing. Ineffective atrial contraction coupled with
left atrial dilation predisposes to stasis of blood, thrombosis (so called
mural thrombosis), and a risk of systemic thromboembolism (e.g., resulting in
