Diarrhoea

Diarrhoea implies increased stool volume and frequency and a change in consistency from formed to semi-formed, semi-liquid or liquid. The examiner should always inquire about the presence of blood and mucus and establish whether or not there is accompanying pain or colic. There is a wide differential diagnosis and the history may give some leads. Some of the causes follow:

  • Stress, anxiety or fear
  • Gastroenteritis - food poisoning (inquire about recent travel overseas - travellers diarrhoea sometimes called 'Bally belly', eating out or simultaneous occurrence in other people living in close proximity)
  • Coeliac disease (atrophy of intestinal mucosa that appears to be caused by allergy to gluten in wheat), lactose intolerance (seen in infants), fat malabsorption (steatorrhoea - the stool is bulky, pale, odorous, poorly formed and difficult to flush) as in biliary obstruction and chronic pancreatitis
  • Infective colitis (caused by bacteria or parasites) or inflammatory bowel disease (Crohn's disease and ulcerative colitis) where blood and mucus can be mixed with the stool
  • Laxative abuse
  • Recent broad-spectrum antibiotic treatment, magnesium-based laxatives, side effects of many other drugs (especially drugs used in treatment of HIV infection)
  • Irritable bowel syndrome
  • Hyperthyroidism (usually together with weight loss despite normal or increased appetite)

  • Acute diarrhoea do not suppress for the first 24-36 hours in adults or 12-24 hours in children
  • Rehydrate patient with good water and electrolytes. Infants dehydrate very rapidly.
  • Avoid coffee, tea, alcohol
  • Avoid Dairy foods
